Systematic review and classification of: Dialectical Behavior Therapy for Adolescents (DBT-A) (1.ed.)

Background: This article is a systematic review of current knowledge of the effects of Dialectical Behavior Therapy for Adolescents (DBT-A) aged 12-19 years in a Nordic context. DBT-A is a manualized and group-based intervention containing 24 weekly sessions, targeting youth with suicidal and self-harm behaviors. The goals of DBT-A are reduction of such behaviors, as well as reduced dropout from therapy. The treatment consists of skills training related to regulation of emotions and behaviors in interpersonal relations, as well as skills in enduring emotional pain. Secondary goals in the treatment is reduction of drug use, anger and depression, and the strengthening of daily functioning.
Method: The following databases were searched in order to find published articles about the DBT-A intervention: Embase, Medline, Psycinfo, Norart, Cochrane, Cristin, NORA, SCOPUS and SweMed, as well as material from the program owner in Norway. The search was evaluated to identify international reviews, and Nordic effect studies related to the intervention. Six publications, including three based on one Norwegian effect study were included in this review.
Results: The results include a review of the description of DBT-A, the outcome of the effectiveness studies, methodological quality of the research, and implementation quality. The results show that DBT-A is an evidence-based intervention with solid a theoretical foundation. The effect study where DBT-A was compared to TAU give reason to believe that the intervention causes reduction in self-reported suicidal ideas and self-harm for youth between 12 and 18 years. The intervention has well described implementation strategies, which strengthens the probability that the intervention is delivered as intended in mental health services. The included studies represent the first effect evaluations internationally.
Conclusion: DBT-A is classified at evidence level 4 – Intervention with satisfactory evidence of effectiveness.
